DO YOU QUALIFY?

Contact your HSA or FSA Benefit Specialist or Administrator to find out if you are eligible to use your FSA/HSA card or be reimbursed for your Guild membership monthly subscription ($40-50). Make sure to let them know the Guild is a weight loss membership. 

DOCUMENTATION

Find out what you need from your health care provider to be elegible. If required, ask your doctor to complete a letter of medical necessity or write a prescription and submit to your health insurance provider if needed.

Join using your FSA/HSA Card.

If needed, print and submit your monthly receipts for The Guild for reimbursement from your insurance company or FSA/HSA account.
